Public health is holding an immunization clinic at the school for Grade9 & Grade 12 students on Oct 24, 2008 . Each Grade 9 studentwill be offered 2 vaccines, one protects against tetanus, diphtheria andwhooping cough and the other protects against 4 types of meningitis. Each Grade12 student will be offered 1 vaccine which protects against Measles, mumps andGerman measles.
Immunization consent pamphlets will be sent home tomorrow (Wednesday).We ask that the “request for immunization” form for each vaccine iscompleted. There is a spot to tick a yes or no indicating whether the studentwill receive the vaccine. Students 16 years of age and older may sign their ownconsents. Students less than 16 years of age must have parent/guardian sign.Please return consents to the school by this Friday.
